His journey to improve quality has traversed many arenas including better patient communication tools (motivational interviewing), creating the first broad scale diabetes registry, designing and implementing a care management training program leading on statewide implementation of the Patient Centered Medical Home across 150 practices, defining the medical neighborhood and the role of centers of excellence. Some highlights of these initiatives include founding the Penn State Institute for Diabetes and Obesity and statewide primary care transformation in Pennsylvania and establishing a national and international voice for new models of care at the ADA, Endocrine Society, and International Diabetes Federation.
Dr. Gabbay’s recruitment to the Joslin in 2013 provided an opportunity to implement, train, and lead care transformation utilizing his research findings to impact care locally, regionally, nationally and internationally. This has included guiding transformation of the Joslin affiliate program to become a broader population health partnership and implementation of core principles from his research in projects globally.
